Supported Living is about enabling and supporting people to live in their own home. There are many different ways to achieve this;
A tenancy in an individual placement
A shared living arrangement
Sharing with a friend
The level of support provided to each person will vary depending upon their individual needs. Havencare is committed to person centered planning and will work with the service users, families and advocates, to achieve the best possible quality of support.
Who is it for?
Havencare can provide this service for all adults (18+) with learning disabilities.
Who will benefit?
People currently living at home with parents/carers who are looking for greater independence.
People living in shared residential settings who would like to move into their own home.
What is the aim of this service
To assist the person to improve their quality of life through process of Person Centered Planning.
To work closely with other agencies involved with the person, particularly Care Managers, health professionals, housing providers and benefits agencies.
To liaise with support and the family and friends of the person.
To improve and increase the involvement of the person in their local community.
